Support Worker - Liaison & Diversion, North Yorkshire Horizons

This is a new, exciting service for Humankind. Led by Tees, Esk and Wear Valleys NHS Foundation Trust (TEWV), Humankind and Spectrum Community Health CIC will provide liaison and diversion services across North Yorkshire. The all age liaison and diversion service, will offer assessment, advice and support for people who come into contact with the criminal justice system and are experiencing mental health problems, have a learning disability or other complex needs. The service will also support staff within the criminal justice system with advice and signposting.

Humankind will deliver the community-based through-care and follow-up element of the service, including peer support engagement, to ensure people are able to access and engage with local services and resources to meet their individual needs. As part of our committed and enthusiastic team we are looking to recruit 2 full time support workers. 

Main duties of the role include:

• Working within a multi-disciplinary team to provide information, advice and guidance, referral and direct individual support to enable access to support services to promote mental and physical wellbeing.

• Delivering time limited support interventions to promote engagement within wider support services.

• Appropriately engaging individuals with peer support and promote engagement with those with lived experience.

• Maintaining strong links with stakeholders including police, court, probation and CRC services, mental health teams, substance misuse services and learning disability providers alongside a range of health and wellbeing services.
