In June 2018 we re-branded from DISC to Humankind and over the last 12 months we have developed a new 5-year strategy outlining our ambitious plans for the future.
To help us achieve these ambitions, we are recruiting for a Temporary 12 months (maternity cover) Full Time Complex Needs Recovery Co-ordinator to be based in our South Tyneside Adult Recovery Service.
This is a newly mobilised service supporting residents of South Tyneside with Drug and Alcohol issues.  The successful post holder will match our organisational ambition and commitment to provide the highest standards of service delivery across South Tyneside, engaging with local communities and supporting their priorities.
Main duties for the role will include:
• To work as part of an integrated team providing collaborative case management and interventions to support service users identified as having multiple complex needs.
• To work effectively with service colleagues, community organisations and stakeholders across the full range of settings to maximise effective and collaborative care and support for individuals.
• To provide holistic and structured recovery coordination and psychosocial interventions to service clients, their families and carers.
• Working to ensure effective delivery of recovery coordination within the full integrated service model, with a focus on successful and sustainable exit from treatment.
