Wear Recovery is a service made up of several key partners including Humankind, enabling the development of a peer led, recovery community. Providing a safe environment in which people can develop the life skills, and confidence to achieve and maintain recovery and abstinence from their substance misuse, successfully re-integrate into society, and become active, contributing citizens.

Based within our Houghton Hub in Sunderland, you will be required to work as part of a team to provide eefficient and effective response to all contacts to Wear Recovery including professionals, service users, family members or carers – this will include contact via telephone calls, emails, faxes and web enquiries.  You will also complete triages for new referrals to the service as well as providing brief intervention over the phone. In addition provide hospital liaison support to adults to enter Wear Recovery service and support them to move through the integrated, recovery oriented service, maximising opportunities for them to achieve sustainable recovery.
The post holder will be expected to work closely with partner agencies in community and outreach settings.
